It is the first time I did this kind of lj thing.
Usually I will just beat them to puppies.

The procedure to do this is actually pretty easy. But of course we should not abuse the system and only do it at a last resort.
I'm also not sure will it be useful at all.
But who cares, when something I think needs to be done, I will just do it.
Giving threats only carries no meaning. Just do it!

Just sharing it here, in case bros need it in future.

Step 1
Asked for the official consultant full name

Step 2
Write your full story in a letter.
Be sure to give as much details as you can, including the dates and even time (if possible) and sequence of events.

Step 3
Print out a hardcopy of the Statutory Declaration form and Letter of Acknowledgement form, both available from smc website.
Both the SD and LOA are simple forms just 1-2 page.

Step 4
Book an appointment with a commissioner of oath.
The fees for getting the oath signed and done is about $30.

Step 5
Turn up to the appointment with the letter of Complaint, SD and LOA forms

Step 6
After the forms are duly filled, scan it to pdf format.

Step 7
Email it to smc
[email protected]

Follow the instruction properly.
The email should only contain 2 attachment in pdf.

1. SD form together with your letter of Complaint
2. Letter of Acknowledgement

Step 8
Completed. Wait for investigation
 
My leecommendation is before doing it, discuss the issue with another party first,
prefably someone you think with good iq

The reason is that, we alone, no matter how smart we are, or we think, may at times blinded by anger and make some wrong or unfair judgement.
So sitting down and discuss it with another person can give us some feedback and opinions.
If both your opinion and the other party is in sync, then you will be rest assured this action needs to be done for justice.
 
Why don't justbsend a complaint email to the hospital feedback unit? Then no need so troublesome so many procedures...
 
Note : For those who are not good or confident to write their own story, the commissioner of oath or fit can help you to write.

This is where the cruz of the problem lies.
Becos people think that it is troublesome, and the senior loctors knew that generally people will find it troublesome, they then misbehave.
So we need to do it when is deemed.

I last time got write email complain before. Then they reply and apologise and waived all fees.

In the letter of Complaint, we are not doing contents that are potentially defamatilory.
But rather a sequence of events that we think the loctor has failed their part..
And we did not published it anywhere.
It is solely for smc to investigate the incidents.
 
Look at the amount of time wasted and also money in order to help them improve their system. The complaint system is designed in such a pernicious way to create as much friction as possible for the complainant in the bid to wear the person down. How many genuine and valid complaints were being filtered off by this egregious complaint system? The medical board should be ashame and take a long hard look at themselves period....

State clearly what happened and don't make any assumptions.
Those things that we mentioned is best to have supportive artifact. Not to indicate items without artifact.

Most of the items I can find artifact in the hospital system, including some that we can access thru healthhub like medical test leesults and xrays, medications etc.
Although the smc investigators can also find it all themselves, is good to display all the home work we have done on our part.

A sequence of what happened should not contain any defamatory content unless we add veges to it.
For eg
After a test is done, and the leesult is this.
And the loctor did not proceed to do his next possible action.
On why the loctor did not perform this next possible action, he will have his own lee son. However when this Lee son is a non acceptable Lee son on your end, you can state it down. It is nothing defamatory.
Then it is upto smc to investigate.
You can be surprise sometime, the Lee son can be as simple as becos they are lazy.
And in the letter, we cannot say becos they are lazy.
For people with lot of money, can they consult a lawyer and issue a letter, instead of going through smc route?

To avoid this potential mistake and backfire to ourselves ,
I have to highlight.
In the letter of Complaint, do not make assumptions and any form of accusations.
The letter should only be a series of events, and on why certain thing is not done when it should or can be done.

In my particular case, it only consists of all these points.

You can ask a lawyer to issue a letter to SMC on your behalf, or if you're sure the Dr fucked it up, to the Dr or his employer.

It all depends on your objective. Sometimes, the action and the objective may not always be the same.

For example, you can throw a wedding banquet, but your objective may not be to celebrate your wedding with your friends or relatives, but purely to earn angpow money.
