If she had done regular, preventive maintenance instead of channelling resources into the advertising space and paying out fat salaries and dividends, we won't need to be on such a tight maintenance schedule today, fixing things on the go and having our trainees work on the tracks even when the trains are running by at full speed. We would be able to space out the maintenance workload rather than cramming it all.

Let's be honest. This episode is just one of the many tragic fallouts from that rot which began years ago.

Another perfect illustration of how some people just deserve to have their lives snuffed out.

Once again the cavalier attitude and poor work ethic of the the lower levels of organisations has resulted in massive inconvenience and heartache for the top echelons of management.

I truly feel for Desmond. He has ended up stressed and distraught through no fault of his own. I'm sure it has affected his family too.

If I adopt the same attitude as those trying to crucify Desmond, I would like to counter propose that the maintenance team who caused all this disruption should be severely disciplined. Post humus convictions should not be ruled out and the families of the deceased should be made to pay some sort of compensation to all those affected.

the technology is there for trains to auto-detect animals, humans or objects on tracks and slow down or apply emergency braking at least a hundred meters upon detection. why operators are not doing it all over the world, i don't know. in tests of driverless vehicles on the road today, they can detect and stop within 30 meters of a possible collision at 60 kmph or 40 mph. trains typically run at 70 mph and require a longer distance to stop. this is especially important for high speed rail in 3rd world cuntries where tracks run alongside and at level with farms, open fields and forested areas with zero fencing. in india and indonesia, i anticipate a lot of dead cows, dead people and horrific collisions.

I don't have to wait for the investigations to be completed. I am aware from personal experience just how dumb the lower echelons of society are.

I worked in manufacturing for more than two decades and the amount of stupidity by peasants that I witnessed had to be seen to be believed.
 
Following Leongsam's analogy, this COULD have been a case where the brothel owner, under pressure from higher ups to cater to some clients, tells the assessor not to wear a condom and see if there is a difference in sensation. The assessor dutifully follows not knowing safety protocols have been violated and he ends up getting HIV.

We won't know who is at fault until the investigations are complete. WHAT we can reasonably conjecture however, is that either the trainees failed to follow safety guidelines possibly due to lack of experience or some other extenuating factor, or they were told to do so by someone higher up. Either way, these are trainees we are talking about, not experienced technicians who may be expected to think things through. Why were they allowed to be put in that situation without stringent oversight? Why were there lapses in safety that resulted in two needless deaths?

yes... the proximity sensor technology is already a standard feature in german cars today. However, it would take longer for a train to slow down or stop due to the momentum -- so it might be pointless to have a sensor anyways...

No matter how many procedures are drawn up, training programs conducted and drills rehearsed, nothing can stop stupid people from doing stupid things.

We had a worker in our factory end up with 3rd degree burns because that clown decided to have his smokes next to a container of highly inflammable industrial solvent.

This was despite the area being clearly demarcated as a NO SMOKING zone with signs of all shapes and sizes, safety barriers and tape marking on the floor.

The factory lost 12 hours of production and management had to spend countless hours dealing with Ministry officials and investigators. After more than 6 months of stressful waiting they concluded that we had followed all the required safety rules.

I curse that fucker to this very day. I'm sure Desmond feels the same way.

I would have expected maintenance workers to be smarter still. Perhaps it is not the case in Singapore.

there are dozens of white papers on wireless and optical sensors in train-collision avoidance being proposed to the industry. they are useful in india as i've indicated as most authors are from india. the sacred cow needs to be protected at all cost. ;)

You do have a point. She has no background in engineering and therefore no appreciation of maintenance and as the years added on the neglect became worse. The fact that the retail front made impressive gains does show where the focus was. She might be the immediate cause but the blame then has to be on the the folks that appointed her. GLC Boards are typically sinecures. Key appointments, targets and direction are typically set by the Holding company, in this case Temasek or the Govt.

I found it odd that the Minister of Transport is now setting the targets, bringing in people that are overseeing its delivery and the current point person. In the scheme things this is not good governance. With the PM's wife being the beneficial owner of this enterprise, no sure if the environment is right in the first place.
