台北市光明消防分隊小隊長李耀文今天早上8點45分駕駛消防水車前往關渡汽車驗車廠驗車，9點10分完成檢驗駛出，沒想到，5分鐘後就在大度路一段，準備停等紅燈，消防車頭疑似因沒鎖好，突然向前傾，他衝出擋風玻璃飛出車外，遭自己駕駛的消防車輾過，送醫不治身亡。
長安派出所及交通分隊初步研判，消防車因為車頭「未卡好」，車頭才會在車輛煞車、準備等紅燈時向前傾，導致小隊長遭甩飛出車外，且經後方車輛監視器顯示，消防車輾過小隊長，車輛依慣性向前滑行，越過路口撞上機車道旁的電線桿才停止。
警方表示，經初步勘驗，消防車頭鎖固處無故障、破損情形，疑似因為驗車後，沒有鎖緊釀禍，將再由相關車輛技師鑑定，釐清整起事故原因。
驗車廠陳姓經理表示，李小隊長今日在車輛檢驗後，獨自將消防車頭鎖上，場內的監視器畫面及驗車資料均由消防局人員帶回，稍後也將前往派出所製作筆錄。
據了解，關渡汽車有限公司是負責台北監理所委託大、小型驗車廠，消防局光明、北投、關渡、石牌等北投地區的消防車輛，長期在此驗車場驗車。
這輛2001年出廠的水車也是普遍使用車型，今天由李小隊長駕駛驗車，經驗車場人員導引車輛進入，進行煞車、排氣等項目檢驗，驗車廠指出，在檢查引擎號碼時，由李小隊長手動掀開車頭，再手動扣回，獨自完成作業。

左轉燈剛亮，這時候路口突然出現了一輛倒頭消防車，整輛消防車車頭倒了下來，幾乎貼在地面上一路滑行，等紅燈的貨車駕駛以及正要轉彎的聯結車看到這副景象，趕緊踩煞車，只看到這輛消防車往路邊的工地方向，直接撞上路邊的電線桿才停下來。

工地員工聽到砰的一聲，外出查看，結果衝到車旁，只看到被撞扁的車頭，駕駛呢？竟倒在路中央，趕緊打119請求救援，受傷的是北市光明消防分隊李姓小隊長，15日上午8點開這輛消防車到北投一家汽修廠保養車輛。
回程行經大度路時車頭突然向前倒，小隊長疑似第一時間就撞壞擋風玻璃，人飛出車外，但車子繼續向前滑行，他根本來不及閃避，直接遭消防車輾過，緊急送醫後仍傷重不治。
台北市消防局第四大隊組長吳志成：「因為只有小隊長一個人，所以現在受傷到底真實的狀況，我們現在沒有第三者可以做一些確認。」
