Yes, the holster strap is the cocking hammer guard. It's not possible for a revolver to misfire while being holstered. In PA, we were trained for single-trigger firing without cocking the hammer first. The trigger would cock the hammer itself first before firing. It's very stiff to do that without cocking first, but we got used to it after a few range sessions.

Without pre-cocking the hammer (which no one is supposed to do in training), it takes an immense amount of finger force to discharge the weapon. It would be next to impossible to do it accidentally unless you were really stupid
And there is almost no way to discharge the weapon in the holster, as the holster strap locks the hammer in place, not allowing it to cock backwards
I strongly suspect that either this joker didnt holster his weapon properly or that he removed the gun from the holster and did something really stupid to discharge it accidentally

correction... for single action revolvers, you need to cock the hammer before you squeeze the trigger; and for double action, all you need to do is squeeze the trigger. i teach firearms handling, and my bad for getting that upside down.
single action revolvers were used in the wild wild west over a century ago. almost all of today's revolvers in active use are double action (da/sa), where you can pull the trigger to do two actions (cock and fire) or you can choose to cock the hammer first before pulling the trigger. in single action only (sao), you have to cock the hammer before pulling the trigger. only in a range with antique keepsakes do folks actually shoot with sao revolvers. double action revolvers going back a hundred years (e.g. colt 45, 1909) already have safety levers.
