Looks to me he has neurological problem unable to anterior flex shoulder and even abduct the shoulder. Bilateral. Usually bilateral more a sign of something systemic.
Maybe he has some C4 C5 C6 brachial plexus injury? Erb's palsy?
It gets better because of the repeated use and flexing of the muscles invovled. Fires the nerves to work. Physio in that sense. Nothing to do with the neck cracking and manipulation.

One of those is severe nerve injury. Not the neuropraxia type but the axonotmesis type. Complete disruption of the nerve. No connection liao. Gap.
Nothing works perfectly. You got some improvement maybe with some small nerve fibres but the main nerve gone means gone.
It could be that boy has a nerve injury bilaterally. Or he could have some endocrine or metabolic disease that causes some Proximal Muscle Weakness. Thyroid condition? Adrenal? Cushing's?
If systemic possible to treat it then the proximal muscle weakness will recover.
Muscle and bone alignment does nothing for Thyroxine and Cortisol levels.
Also be aware of vertebral artery dissection when going for neck manipulations. I have seen cases at the ED before.

Abstract
The differential diagnosis for ischemic central nervous system infarcts in young patients includes paradoxic emboli through cardiac shunts, vasculitis, and vascular trauma. We report a young woman who developed headache, vomiting, diplopia, dizziness, and ataxia following neck manipulation by her chiropractor. A computed tomography scan of the head revealed an infarct in the inferior half of the left cerebellar hemisphere and compression of the fourth ventricle causing moderate acute obstructive hydrocephalus. Magnetic resonance angiography revealed severe narrowing and low flow in the intracranial segment of the left distal vertebral artery. The patient was treated with mannitol and a ventriculostomy and had an excellent functional recovery. This report illustrates the potential hazards associated with neck trauma, including chiropractic manipulation. The vertebral arteries are at risk for aneurysm formation and/or dissection, which can cause acute stroke.
Vertebral artery aneurysms and dissections are known complications of spinal manipulation procedures (1, 2). An estimated 1 in 20,000 spinal manipulations results in a vertebral artery aneurysm/dissection and ischemic infarct, but the exact incidence of this complication is unknown (3).
That's probably much higher incidence than covid vaccine complications.

Nerve has both motor and sensory when it comes to the musculoskeletal system. To be specific motor, sensory (touch, pain) and propioception (position sense). When doing neurological exam for the limbs got to test for all 3. Also have to differentiate muscular/joint disorder vs nerve disorder when it comes to limitation of movement in joints.
The human body is very complex. Plus the perception of the person to various stimuli is variable. Higher tolerance of pain in some, lower in some. This makes for very complicated treatment of pain.
