Besides drain covers, cycling too near the kerb is a bad idea because there are a lot of small debris like broken glass, metal shards etc left there or missed by the sweeper. Bad for tubeless tires.

User error. Nobody forced him to ride so close to the drain gratings. He must have been dreaming and drifted towards the drain.
I hope he wasn't wearing one of his good watches when he got thrown off!
It certainly isn't PUB's fault. Boss Sam should just move on.
