A 75-year-old Singaporean doctor has been charged in the State Courts with the death of a patient, allegedly caused by tablets he had prescribed.
Haridass Ramdass, who operated from a clinic in Chander Road in Little India at the time of the incident, is accused of prescribing 10 tablets of methotrexate (MTX) without first arranging for Mr Savarimuthu Arul Xavier, 28, to undergo the required tests before prescription.
MTX is a chemotherapy agent and immune system suppressant. The dosage he prescribed was also not in line with established guidelines, according to the charge slapped on him earlier this month.
The case is apparently a first in which a doctor is charged with causing death by a rash act under Section 304A(a) of the Penal Code.
Mr Xavier, a foreign national, was treated by Haridass on Nov 24, 2014, at Tekka Clinic Surgery, where he was given an injection of dexamethasone, a steroid used to treat ailments such as allergic or breathing disorders and skin conditions.

In addition to MTX, Mr Xavier was also prescribed prednisolone - a medication for treating certain disorders and conditions including cancer - and chlorpheniramine, which is used to treat the symptoms of allergic conditions.

The "rash act" of prescribing MTX is alleged to have caused Mr Xavier to develop neutropenia, when the body does not have enough neutrophils, an important white blood cell that fights infection.
He also developed mucositis, a complication of some cancer therapies in which the lining of the digestive system becomes inflamed.
These developments resulted in Mr Xavier contracting "an invasive fungal infection" that led to his death, said the charge.

According to information on the Internet, Haridass has been a general practitioner for 44 years and got his medical degree from India's Karnataka University in 1971.
He is out on a $10,000 bail and the case is scheduled to be raised in court in two weeks' time.
A person convicted of causing death by a rash act not amounting to culpable homicide faces up to five years' jail, a fine or both.

Another Drug Company Recalls Tainted Losartan

In the ongoing saga of contaminated sartans, Macleods Pharmaceuticals Limited has recalled one lot of losartan potassium and hydrochlorothiazide combination tablets (100 mg/25 mg; 90-count bottles).

The recall, to the consumer level, stems from the detection of the probable carcinogen N-Nitrosodiethylamine (NDEA) above interim acceptable daily intake levels released by the US Food and Drug Administration (FDA) in the active pharmaceutical ingredient (losartan) manufactured by Hetero Labs Limited.

To date, Macleods has not received any reports of adverse events related to this recall.

The recalled lot has a national drug code (NDC) of 33342-0052-10, a lot/batch code of BLM715A, and an expiration date of July 2019.



The recalled medicine was distributed nationwide to Macleods wholesale distributor and retail customers. The company is notifying its distributors and customers by phone and/or in writing via a recall letter to immediately discontinue distribution of the specific lot being recalled and to notify their subaccounts.

Macleods is arranging for return of all recalled products to Qualanex. Customers with questions regarding this recall can send an email to [email protected] or call 888-280-2042 (7:00 AM to 4:00 PM CST Monday to Friday).

Contaminated sartan products first surfaced in July 2018 and since then several drug companies have recalled sartan products, as Medscape Medical News has been reporting.

Most recently, Torrent Pharmaceuticals Limited expanded its voluntary recall of losartan potassium and hydrochlorothiazide combination tablets to include six more lots, which means that 16 lots of losartan-containing medicines recalled since December.

As with the Macleods recall, the Torrent recalls stem from the detection NDEA in losartan manufactured by Hetero Labs Limited.

The FDA is keeping an updated list of losartan medications under recall on its website.

Adverse reactions or quality problems associated with the use of sartan products should be reported to FDA's MedWatch Adverse Event Reporting Program.

The tainted sartan problem is not limited to the United States; it has spread to Europe and other countries, including Japan.



Earlier this month, Fierce Pharma reported that Pfizer's Tokyo-based Japanese subsidiary is recalling from the country five lots (760,000 tablets) of valsartan and amlodipine besylate salt after the active pharmaceutical ingredient made at a plant in India was found to contain trace amounts of NDEA and N-nitrosodimethylamine (NDMA), another probable carcinogen.
