They had only stated the fact that: "In the last 15 years of SSPH operations, there has not been any reported injury of servicemen due to the gun lowering for maintenance or operating in or firing of the howitzer." What is wrong with this statement, which is after all, also a truth? Until now and even before the verdict from the COI, did they at any one time explicitly said that Aloysius' death was caused by his own actions? Furthermore, if this is still not enough for you, it has been stated that: "None of the COI members work within Mindef or are Singapore Armed Forces regulars." What more do you want?

SINGAPORE: The Singapore Armed Forces (SAF) will set up an Inspector-General's Office to ensure that "command emphasis on safety" is applied and enforced across all SAF units, said the Ministry of Defence (MINDEF) in a news release on Thursday (Jan 31).
The office will support the Chief of Defence Force Lieutenant-General (LG) Melvyn Ong and have full authority to scrutinise and enforce safety processes and practices at all levels of the SAF.

LG Ong held a command call at Pasir Laba Camp earlier Thursday, where he met with 750 active and operationally ready National Service commanders and shared plans to make training safer for SAF servicemen.
Speaking to media after the command call, Chief of Army Major-General (MG) Goh Si Hou said that the army will expand the number of safety inspection teams and full-time safety officers to conduct checks and audits on units' safety systems.
This was part of the army's effort to strengthen safety management structures on the ground, MG Goh said.

MG Goh also said that the safety time-out that was implemented across the army in the wake of actor Aloysius Pang’s death will be lifted progressively after Feb 7.
All army units will be allowed to resume basic types of training such as physical fitness training, small arms live-firing, Individual Physical Proficiency Test (IPPT) and IPPT Preparatory Training (IPT) for NSmen from Feb 7.
Corporal First Class (NS) Pang, an armament technician with the 268th Battalion, had been injured while conducting repair work on a Singapore Self-Propelled Howitzer.
The 28-year-old had been participating in Exercise Thunder Warrior, a live-firing exercise, in New Zealand when the accident happened.
Pang died on Jan 23 after undergoing a number of operations to treat his injuries.
“Safety is a command responsibility. Commanders answer for the training and safety of their men. To do so, commanders have to be fully committed and personally and intimately involved in their unit’s training, operations, and safety,” said LG Ong, as cited in the press release.
“The reduction in training tempo will allow us commanders to take stock, re-orientate, and give full attention to this,” he added.

MINDEF confirmed on Wednesday that a Committee of Inquiry (COI) was convened on Jan 25 to investigate Pang's death, with a judge from the State Court as its chairman.
Other members of the COI panel include a consultant medical specialist, a member of the External Review Panel on SAF Safety, a member of the Workplace Safety and Health Council and a senior-ranked national serviceman.
None of the COI members work within MINDEF or are SAF regulars, the ministry said.
The incident in New Zealand is the fourth time in 18 months that an SAF soldier has died while training.
Minister for Defence Ng Eng Hen is set to deliver a ministerial statement at the next parliamentary sitting in February to address the deaths.
The command call was also attended by Chief of Air Force MG Mervyn Tan, Chief of Navy Rear-Admiral Lew Chuen Hong and all senior commanders of the SAF.

SINGAPORE - A Committee of Inquiry (COI), chaired for the first time by a State Court-nominated judge, was convened on Jan 25 to look into the circumstances leading to the death of Singapore actor Aloysius Pang while he was on reservist duty, the Defence Ministry (Mindef) announced on Wednesday (Jan 30).
Defence Minister Ng Eng Hen will also deliver a ministerial statement addressing recent national service training deaths when Parliament next sits on Feb 11.

Since June last year, Mindef has included serving or former judges as people eligible to chair the COI, in addition to senior civil servants.

While the statement did not name the members, the ministry said that the other members of the independent COI are a consultant medical specialist, a member of the External Review Panel on Singapore Armed Forces Safety (ERPSS), a member of the Workplace Safety and Health Council, and a senior-ranked national serviceman.

None of the members work within Mindef or are SAF regulars, the ministry added.
Corporal First Class (NS) Aloysius Pang was doing repair work inside a Singapore Self-Propelled Howitzer (SSPH) during an annual live firing exercise in New Zealand on Jan 19 when he sustained a serious injury that ultimately led to his death four days later, on Jan 23.
The operationally-ready national serviceman (NSman) was an armament technician from the 268th Battalion, Singapore Artillery.

His death is the fifth reported since September 2017, following four preceding years of zero training- and operations-related fatalities.
The COI, whose findings will be made public, has full powers and access to material and witnesses to investigate the circumstances leading to the death.
It will also determine the contributory factors and make recommendations to rectify any lapses uncovered.

The COI, which was convened by the Armed Forces Council on Jan 25, will submit its full report to the ERPSS for comments.
Mindef on Wednesday also provided details of the safety record for the SSPH and the standard operating procedures required of its operating and maintenance crew.

It said that in the last 15 years of SSPH operations, there has not been any reported injury of servicemen due to the gun lowering for maintenance, or operating in or firing of the SSPH.
In the same period, more than 1,000 servicemen, NSmen and regulars have been trained to operate the SSPH, with around 12,500 rounds fired.
Chief of Army Major-General Goh Si Hou said at a press conference on Jan 24 that Mr Pang was in the SSPH cabin with two other servicemen - another technician and a gun detachment commander - when the incident happened.
Initial findings showed that Mr Pang suffered crush injuries when he was unable to get out of the gun barrel's way as it was lowered.
When maintenance work is required during training and operations, a team of technicians will be activated and they will take charge of the SSPH.

According to the information released by the ministry, the technicians may require the assistance of the gun commander. The army technicians are also trained and required to abide by the same drills and safety protocols as the four-man crew that operates the SSPH.
Mindef added that the SAF has reduced the training tempo for commanders and soldiers to assess safety protocols and plans in their units.
"The reduced training tempo will remain in place until the SAF is satisfied that training and other activities can be conducted safely. The SAF is committed to strengthening the safety culture on the ground," it said.

Instead of being just a keyboard warrior who uses Aloysius' misfortune and death here as another opportunity and means to condemn and back stab the government, I wonder how many of you had showed your real concern and sympathy by at least paying a visit to his wake to bid him a final farewell? Although I don't know him in person, but I still made an effort to do so, together with my wife. This is that much we can do for him.

My conclusion is if it is eventually due to any foul play by his team mates, those found negligent must be dealt accordingly by the law. If it is due to Aloysius own mistakes, then he had indeed overpaid with his life. If it is due to insufficient safety procedures from MINDEF, then they had better revise all their safety protocols on all their machines and training programs so as to further minimize more (preventable) accidents from happening in the future.
