This has to work both ways.
Obviously this chap has forgotten to ask the question whether our hospitals are willing to pay the "right amount of $$$" to hire S'poreans even if they are fit and willing to do the job.

Ngiam assumes that sinkees don't want those jobs. The reality is that they want them but the hospitals don't want to pay the living wage ...so they have resorted to hire foreigners. Close the door to the foreigners is the only solution to force the hospitals to hire sinkees and paying the living wage.
And he has so little faith in sinkees.
