A Secondary Two student was bitten by a cobra after a three-day leadership camp at Lim Chu Kang last Sunday (30 Oct).

While waiting for his school bus near the camp site, Andrick Kok from Mayflower Secondar School, felt a sharp pain on his leg. He looked down to discovered that he had just been bitten by a snake. “It was angry – it sort of glared at me, hissed and then slithered away quickly,” Kok told the New Paper, adding that the snake only let go after he shook his foot vigorously.

An instructor at the camp site rushed him to the sick bay area. A tourniquet was tied over the student’s left leg to prevent poison from spreading. He was later rushed to NUH, where he was hospitalised.

By Monday, Kok had passed the recommended 24-hour observation period without displaying major symptoms.
 
Was there many a time for NS training and even sleep in the bushes. Only incident I know was in SAFTI when my platoon commander got stung on the lip by a hornet. He look so handsome after that.:D

The soldiers that were shot, maimed and killed, not by cobra or some other creatures, but from stupidity on either their part or by others.:(
 
Emergency First Aid is to 1st tied the limb near the bite but above the bite.

Say toe is bitten by snake then tie near ankle.

Then use a heated and satirised needle or spike to puncture around the bite area and squeeze out the poison venom ASAP & as much as possible. Puncture depth have to be as deep as the snake's fangs. Discharge the venom.

Use the Chinese vacuum cups to suck out the venom, on the way to the hospital. Serum injection will be needed.

毒蛇咬伤,紧急放血拔罐
经常在新闻媒体的健康医疗版上,看到读者询问毒蛇咬伤的急救问题,也分别看到中西医师的答复。
笔者虽然未曾有过机缘,亲眼目睹,或是现场急救处理过毒蛇咬伤的案例。不过,倒是曾经放血拔罐,急救处理过三次虎头蜂螫伤的案例,方法简便、易学,一次见效,委实快速、显著而且安全。
毒蛇的毒性,超过虎头蜂。但从中医辨证、论治、治则、机转的理论与临床思考,毒蛇咬伤的急救方法,应该可以比照虎头蜂螫伤的急救处理,同样可以快速缓解、排除蛇毒伤害,更进一步减少生命的威胁。
台湾每年四月至十月,是蛇类出没的旺季,也是蛇咬伤的常见时期,尤其是七、八、九月的三个月,蛇伤案例较多。遭遇毒蛇咬伤,所知的对策,只有紧急送医、注射蛇毒血清一途,几乎别无他法。
笔者雅好登山,登山装备当中,绝对不可或缺的配备,就是针灸及放血拔罐用的器具。攀登深山,面对野外突发状况,一切医疗卫生,自救救人,都靠自求多福。笔者幸亏略具针灸经络、放血拔罐等家传的传统疗法,器具简便,携带容易,经常可以发挥当场急救、适时医疗的效果。放血、拔罐,用于虎头蜂螫伤及毒蛇咬伤,同样适用,值得参考。
突然遭到毒蛇咬伤,由于毒性一时难以判定,所有的方法,包括包扎、后送就医、辨明蛇的种类、注射血清……,其实都是缓不济急,或是止于消极的消炎、止痛、解毒;万一路途遥远,运送不易,延误医疗的「黄金时间」,必将造成无可挽救的悲剧!
以虎头蜂螫伤为例,患者局部皮下组织、肌肉遭受攻击,如果能够在最短时间之内,及时将蜂毒设法逼出、吸出,儘速设法将毒液减量、排除,绝对是最正确、最有效的治疗原则,也是最贴切的急救方法。放血、拔罐,可以立即排除、减少皮下肌肉组织中的毒液,正是最为简便、直接,而又是最为积极、适当、速效的一种技法。
毒蛇咬伤,咬入的部位,无论皮下或是肌肉,综合西医的见解,主要的处理原则,通常包括:绷带包扎,阻止毒液漫延。或是带血清上山,作静脉注射或肌肉注射,降低全身性的反应发生。或是使用吸具,吸出毒液,不过效果大约仅只1-30%的效果。而中医使用「黄连解毒丸」,可以内服、外敷。也仅只能够消炎、止痛,并无解毒功效。
现有的中西医疗法,同样都是属于事后的、消极的支持性疗法,对于可能致命的中毒病情,只怕于事无补。
其实,毒蛇咬伤的治疗,只要比照虎头蜂螫伤的疗法,赶紧採用放血、拔罐的方式,快速吸除毒液、「稀释毒液」后,毒液的威胁,当场就会减到最低。
放血的方法,原则是要避开血管神经。先用放血针(注射针头或採血片均可),在咬伤部位,依据范围大小,常态消毒后,针刺一至数处,深度0.3-0.5公分深。然后,用真空拔罐器吸拔,时间3-5分钟。然后放鬆拔罐器,擦拭流出来的血液和组织液。接著再重新拔罐。
连续数次,出血量不妨稍多,遭毒蛇咬伤部位的组织,可以快速「稀释毒液」、吸除毒液,自然可以有效避免蛇毒的威胁。毒蛇咬伤,尤其是神经性毒液,很容易导致呼吸衰竭,在短时间内死亡。后送就医注射血清,只恐费时费事,延误救治,回天乏术。
放血、拔罐的疗法,现场当下立即急救,让毒液在浅表部位,第一时间内,即行吸除,或是至少予以稀释、减量,应该就可以确保伤者的生命安全无虞

I was first aid instructor with St John's ambulance brigade for some 15 years but 20+ years ago when I was in school.

In my NS days I used to kill cobras 20-30 of them. Especially during exercise or night prowler duties. Many black cobras in SGP, Sembawang Camp Changi Camp SAFTI exercise areas. Usually 1 meter size. I chase them up and accurately step on their head to squashed them. Larger size can not do this trick. SAF boots not powerful enough.:D

In SAFTI live fire hills I saw a huge black cobra, the thickness was like adult upper arm 3+ meters. It crossed our dirt path in front of our landrover, I got off the veh to look for it but not found. There must be a hole it got hidden. Snakes not so risky there as SAF blind bombs, got countless number of mortar shells around.

For 1st Aid when medical help is too far away and patient could died of heart failure before reaching hospital. Can consider to BURNT the WOUND. Open the wound after tied to stop bleeding. And press hot burning metal against the area containing venom. The venom is bio-toxin meaning it is also protein, and it can not withstand heat. You cook it and protein will disintegrate into something else less lethal.

Burn wound is usually not lethal, far less lethal than snake venom. So if hospital is few days away, you rather have a bad 3rd degree burn rather than a corpse of with failed heart, burn the snake bite. The snake bite is already more painful than the burn itself anyway. :p Ouch!

Any problem using the mouth to suck out the poison as long as you don't swallow the poison?
 
it is normal for the snake to bite a sinkie, everything hates sinkies... it knows how ball-less sinkies are...

Ah! the women have already done that, suck the 'posion' from the 'snake'...many don't swallow the 'poison'...ha ha ha ha. Of course, you suck the poison out & spit immediate...but this is dangerous, for some snake poison react instant...does a swig of whisky counther the poison?

You didn't read the report?, obviously they have no experience, I would not wear a pair of slippers in a grassy area, a covered up boots or canvas shoes. Luckily it was a small cobra, I have encountered a large cobra when I was young, I ran first, ask later!

Surely unsafe but if you care say your child's live is at risk then you will do it. :p Spit out the venom and wash mouth lah. If say your own arm is snake bitten then, you may suck your own wound.

Not as bad as the bite, when venom is in mouth it will get into the gum or dental wound or openings. Spit and gaggle ASAP if needed to use mouth. In jungle it is the easiest and most available means to save a live and some situations are really no choice.

The vacuum is no difficult to do, as long as you have container or cup and flame. Say for the boy's case they were camping and at least would have some of these helpful resources to save him.

I now remember one incident. I was walking my dog 20 yrs ago when it was a puppy in Alexandra Park area, where I used to live. My dog sniff everywhere it goes as usual, and it found a black cobra also 1m approx. The idiot went forward to face the cobra which already stood up with neck expanded. That was the only time I had seen that other than in zoo or India's snake-man performance. The dog went nose to nose approx 30-40cm with the cobra.

KNN! I was also wearing slipper.

And I had to drag the leash or the dog will go further forward. So KNN I took off slipper & threw at the cobra, 1 miss 1 hit, the cobra vanished into bush. When I picked my slipper my dog still went to look for it's new friend, KNN! I knew it's nose is able to find it, but WAF! I was no wearing boots to kill it, and don't need a dead puppy. So I drag my dog and forced it to leave.

It was like 6:45pm getting dark.
 
One more incident was Pulau Ubin in rubber tree estate. I was hiking with ex-GF.

It was quiet, and then some sound came ahead like leaves of rubber tree were sweep by something moving.

I follow the sound and saw it, black cobra about 2 m. GF panicked. And that one was too big to be killed using my feet. Can not remember weather wearing boots, but any way even got boots won't be enough to kill that size.

I tried to reach for my Parang in my backpack, but it was jammed and stuck inside. KNN!

Boh Pian can not kill it liao. So I picked up rocks & fragments of coconut shells to throw at the cobra, it fled and vanished in less than 2 mins.

The time was like 10+am but in rubber plantation it was dim.

Naughty boy, you want to find a big thick rubber tree trunk or tree trunk, to cannoodle & rubba rubba...the snake beats you to it...did you repent?

Nowadays the mata, got pistol, but bo 'balls', they tell you to call AVA. Ramseth, 'eat snakes' that is the best snake experiences he can tell you, the time he spent sleeping in places you would never know, you can sleep in.:D

Interesting account Uncle Yap... Had a close shave while walking the dog too.. Only few months back my dog saved my ass while I walking him around my mom in law's neighbourhood at Changi's Mariam Way.. Was wearing slippers during that night when I nearly stepped on to a small but fierce black snake hissing at me menancingly.. My dog next instinct was to immediately pounced on to the snake but I pulled him away in time before that small black bugger slithered away into a longkang.. Once I was driving in the middle of the nite on the road of the old Changi Hospital... Was drizzling lightly that night when suddenly I slowed down as I thought I saw a long rope on the middle of that road..:eek: Curious, I braked my car, parked it at the side of the road, stepped out of my car and walk towards that ''rope''... And to my horror that ''rope'' was a long 2m python.. Terrified I quickly ran back to my car and waited for the snake to slither inside the longkang besides a bus stop...
