花蓮慈濟爆出離譜醫療疏失！院內護理人員將一病患尿液檢體誤為生理食鹽水，混入白蛋白打入另一名肝硬化病患體內，病患不幸於10多天後過世，家屬不滿院方第一時間隱瞞未告知，且說法一變再變，事後態度傲慢且未給家屬真相。

當成食鹽水 混入白蛋白
家屬蔡先生表示，68歲的父親長期有肝硬化問題，上月下旬例行性回診，當時父親是走進醫院，但醫師認為父親的肝指數偏高，因而收治住院觀察，當天晚上即安排為父親打白蛋白，就發生了這起離譜疏失；原本家屬並不知情，大約6天後，院內的護理人員實在看不過去，才向家屬透露此事。
蔡先生說，事後醫院從未主動告知，家屬得知後向醫院提出質疑，院方才坦承此事，但說詞反覆，從一開始承認有尿液打入，之後又說有趕緊回抽，到最後變成尿液壓不進去，所以尿液應未進入父親體內，令人為之氣結。
家屬控訴 院方說詞反覆
蔡先生質疑，院方事後預防性為父親施打大量抗生素，如果沒打入尿液，院方為何緊張到要打抗生素，且家屬要院方提供父親的血液培養報告，院方一直藉故不給，家屬要的不是賠償金，而是一個道歉、一個真相，花蓮慈濟是花東重要的醫院，希望此一事件，能讓院方面對醫療安全問題，不要再有如此離譜的事件發生。
醫院指家屬索賠數百萬
花蓮慈濟醫院院長林欣榮表示，這起事件發生在小夜班，護理人員在誤注射檢體尿液後，因阻力甚大，無法順利推入管路內，同時間另一護理人員察覺，立即反抽並拔除及置換點滴管路，所以尿液有可能並未進入患者體內，如果有，量應也不多。二位護理人員隨即通報值班主治醫師，並進行兩套血液培養，接著給予預防性抗生素。

林欣榮指出，事發後，護理部隨即啟動調查程序，並進行系統性的檢討改進，血液培養亦完全未長菌，病人並未因此感染。病人長年因肝硬化於院內就醫，本次入院時呈現急性肝衰竭，且有嚴重腹水，主治醫師當時即告知家屬，病情極為嚴重，希望家屬有心理準備。
林欣榮強調，院長室主管在得知後即介入了解，並會同護理部主管向家屬說明原委並誠懇致歉，且主動吸收全部醫療及看護費用，但家屬提出精神賠償及維護醫院名譽達數百萬賠償，院方實在無法接受，將經由第三方鑑定，釐清因果關係。
