LTA's incompetence shows up in COI - running on auto-pilot itself?

Confuseous

SINGAPORE - In its report yesterday, the Committee of the Inquiry (COI) had called on the Land Transport Authority (LTA) to maintain effective oversight as regulator.

Observers agree there are areas of improvement that LTA can work on.

The COI had concluded that the massive MRT disruptions last December could have been prevented if SMRT's maintenance regime was up to the mark, such as making sure that claws do not get dislodged, and as regulator, the LTA should play its part.

The COI's report stated that in 1995, SMRT had brought the issue of claw dislodgements to the LTA because it needed to seek the regulator's approval for the use of a steel cap on the fastener and claw.

Instead, LTA directed SMRT to monitor the situation for system-wide implementation but there was no subsequent follow-up.

So LTA is to blame. Not SMRT then.

4 July 2012
FOR IMMEDIATE RELEASE SMRT Statement on 4 July 2012
1. SMRT has studied the Committee of Inquiry (COI) report.
2. The December incidents were caused by a confluence of factors, a key one of which was isolated and latent material defects, in particular, in a third rail fastener, and the coincidental existence of two adjacent insulators with defects. The various factors caused a claw to drop, the third rail to sag and the situation to aggravate. The 17 December incident is in turn a result of the 15 December incident, due mainly to one or more possible "rogue trains" that suffered not easily detectable CCD shoe damage from the 15 December incident causing claws to drop and the third rail to sag.
3. SMRT has operated a comprehensive maintenance regime in the past, regularly validated by LTA, which has served us well and placed SMRT amongst the top performing metro operators, as evidenced by international benchmarking studies and testimonies from the experts at the COI hearings.
4. Clear challenges arise from the aging of the MRT system and higher strain from increased train runs to cater to rapidly increasing ridership. Whilst SMRT has been trying to address this, quite clearly, there are areas we can improve in our maintenance and monitoring regime and our work processes.
