1) General incompetence, arising from complacency.
2) Top guns, gaji buta - choh boh, get big salaries, bonus, do fcuk all.
3) Fish rots from top - underlings exploit superiors' lack of interest. Underlings do not mean low lives - think SLA and you know what I mean.
4) Individuals not named - only departments etc
5) Each guilty party promise to do better when caught - but with no name, no shame, incidents get repeated.
6) Tarman acknowledged incompetence in parliament today - back to same story - high salary does not equal competence or clean bill of health.

Ya, PAP style. For a problem, they need to use several high level words to confuse and make you feel dizzy. Just like a worm spotted in a tree and they tell you everything about the roots and soil and then how the tree formed. Than the cost for all those papers and words come out to be million dollars tax payers wasted. End of the day, the tree will still be left unattended until another bugs appear.
