Today, Mr. Liu, a recoverer, was discharged from the Shanghai Public Health Clinical Center wearing a mask and explained to the Beijing News reporter his infection and recovery process. According to Mr. Liu, he visited a teacher in his hometown of Northeast China during the Chinese New Year, and his teacher's son was diagnosed with new coronary pneumonia on February 6. Mr. Liu took the initiative to go to the hospital for investigation. He was diagnosed on February 7 and was admitted to the hospital for treatment. After receiving 7 days of treatment, Mr. Liu was evaluated by the expert group and found that he met the criteria for de-isolation and discharge from confirmed cases.

Mr. Liu told the Beijing News reporter that he had visited the teacher's home for about 20 minutes. At that time, the teacher's son had left the house and had no direct contact. The teacher's son stayed in the parents' house for only one night, and he had no symptoms before being diagnosed.

According to Mr. Liu, the teacher's son had attended a conference in Beijing and many participants were diagnosed afterwards. Mr. Liu admits that no symptoms were found before the diagnosis. After visiting the teacher, he was informed of the relevant situation and was very worried. He decided to go to the hospital for investigation.

"The treatment is still very smooth. I am very fortunate to have recovered. After recovery, my serum may be helpful for the treatment of other patients, so I am willing to donate my serum." Mr Liu told reporters.

Another 28 cured patients were discharged from Shanghai, and some cured patients offered to donate plasma

2020-02-14 17:02 North Vision New Vision Comprehensive TF019

Another 28 patients were discharged from Shanghai on the 14th. This is the largest number of patients recently discharged from Shanghai. As of press time, 90 patients in Shanghai have recovered. In addition, according to the latest report from the Shanghai Municipal Health Commission: No new confirmed cases in Shanghai at 0-12 on February 14th.

Regarding the issue of plasma of the rehabilitated people, Lu Hongzhou, member of the expert group of the National Health and Health Commission and secretary of the Party Committee of the Shanghai Public Health Clinical Center, said that six of the 14 patients admitted to the hospital were willing to donate plasma. Blood samples will then be collected within two to four weeks depending on the patient's physical condition.

"At present, plasma therapy may be valuable for patients, especially critically ill patients. Today's batch of discharged patients have offered to donate blood, which is very moving to me." He believes that the practice of recovering patients' blood donation to save people will Bring positive effects to treatment.

According to reports, on February 8th, based on the new crown pneumonia diagnosis and treatment plan (the fifth edition of the trial) as a guide, the first phase of the new crown special plasma treatment for 3 critically ill patients was carried out at the First People's Hospital in Jiangxia District, Wuhan, and is currently being treated with subsequent hospitals. More than 10 critically ill patients. Clinically, after 12 to 24 hours of treatment, the main inflammatory indicators in the laboratory decreased significantly, the proportion of lymphocytes increased, key indicators such as blood oxygen saturation and viral load improved, and clinical signs and symptoms improved significantly.

According to Lu Hongzhou, one patient in Shanghai is preparing to use the plasma of the recovering patient. "I look forward to the fact that plasma therapy can play a role in critically ill patients, especially critically ill patients."

Mr. Liu, who was discharged from the hospital this time, said, “It took me only 7 days from hospitalization to discharge. Thanks to all the medical staff for their care and care.” After learning that the plasma of the cured person could work for the critically ill patients, Mr. Liu signed up immediately. , I hope to give my own strength to treat other patients or help scientific research.

Shen Yinzhong, who returned home from Geneva on the morning of the 14th, is the director of the Medical Department of the Shanghai Public Health Clinical Center. This time, as the only expert in Mainland China, he participated in the formulation of the WHO clinical management guidelines for new coronavirus infections.

He told reporters, "We can understand the public's concern and expectation for plasma exchange, but this method is not uncommon. Many infectious diseases have been used before, and we have considered this method before." Shen Yinzhong said, Plasma The therapeutic effect of replacement on new coronary pneumonia has yet to be further observed. "It may have a certain effect, but don't expect to use a certain method to straighten new coronary pneumonia at once, or use comprehensive treatment as the principle."

On the evening of February 13, Zhang Dingyu, the director of Wuhan Jinyintan Hospital, stated that the Jinyintan Hospital is in the process of recovering patients' plasma during infusion, and it has also shown some preliminary results, because there are a large number of comprehensive antibodies in the patients during recovery to fight the virus.

This is also the first time since the outbreak of the new crown pneumonia outbreak that first-line medical experts have confirmed that "serum therapy" is used for clinical treatment.

In fact, in the fourth edition of the National Health and Health Commission's trial version of "Diagnosis and Treatment of New Coronavirus Infected Pneumonia," it has been officially proposed that treatment of severe and critically ill patients "can be used in the recovery period of plasma treatment."

The idea of using plasma transfusion for patients in rehabilitation for clinical treatment has actually existed during the fight against SARS.

Academician Zhong Nanshan had put forward a suggestion for treating SARS patients with the serum of recovered people at that time, and Tiantan Biological had also developed the SARS antiviral serum. Some hospitals in Hong Kong, China and Taiwan have also developed serum therapy for patients with severe SARS, and have achieved good results.

After the outbreak of the epidemic, the medical community was struggling to find medicines for the treatment of new coronavirus pneumonia, and many industry experts had suggested to interface journalists that the use of plasma transfusion therapy for patients in rehabilitation was an option.

To put it simply, there is currently no specific medicine for neocoronavirus pneumonia, and after the patient is infected with the virus, the autoimmune system will respond to the neocoronavirus and generate corresponding specific antibodies. When these neocoronavirus specific antibodies have sufficient scale , It is possible to defeat the new crown virus in the body.

In the blood of rehabilitation patients, there is often a large-scale neocoronavirus-specific antibody. By extracting their blood and processing their serum to a certain extent, the antibody in the blood of the rehabilitation patient can be given to the clinic through blood transfusion. Patients, helping patients with their own immune system to fight the new crown virus.

The vaccine that the public is concerned about is actually a traditional inactivated vaccine. It is also used to vaccinate the killed virus to the human body, and then the immune system produces corresponding antibodies. In contrast, the use of serum for rehabilitation patients is more direct. .

"China Bio" also announced on the evening of February 13 that it has completed the collection of plasma from some recoverers and carried out the preparation of neocrown virus-exclusive plasma products and exoglobulin. After rigorous blood biosafety testing, virus inactivation, and antiviral activity testing, special immune plasma has been successfully prepared for clinical treatment.

On February 8, using the new coronavirus-infected pneumonia diagnosis and treatment program (the fifth edition) as a guide, the first phase of the new crown-free plasma treatment of 3 critically ill patients was carried out in the First People's Hospital of Jiangxia District, and is currently being treated with subsequent hospital More than 10 critically ill patients. Clinically, after 12 to 24 hours of treatment, the main inflammatory indicators in the laboratory decreased significantly, the proportion of lymphocytes increased, key indicators such as blood oxygen saturation and viral load improved, and clinical signs and symptoms improved significantly.

But this method is not a panacea, and there are still many restrictions.

Dr. Hong Zhi, the co-founder, president and CEO of Tengshengbo Pharmaceuticals, previously said in an interview with the Rubik's Cube that using the recovered patient's serum to treat other infected patients was a common method to combat multiple infectious diseases in the past, but this There are also some dangers in this type of method, such as inconsistent serum antibody levels in different cured patients, and the inactivated serum still contains some other potentially dangerous sources.

The Pasteur Institute of the Chinese Academy of Sciences also previously told the media that although serum therapy can be more effective in combating or preventing diseases caused by viruses or bacteria, the composition of the serum is complex, and there are still uncertainties about whether it will cause other problems, let alone its It exists for a relatively short period of time and requires a large amount of serum. However, through modern methods, the blood of the healer can be analyzed to obtain a fully human monoclonal antibody sequence that can effectively neutralize the virus, in order to treat and prevent the disease caused by the virus.

Hong Zhi also mentioned that now there is a better way to use the blood of patients to heal, that is, to extract B cells that have memory of the virus from the patient's blood, and then to isolate the antibodies produced by these cells that can fight pathogens It can be used to treat other infected patients.

This is a relatively safe strategy compared to direct treatment with serum, because antibodies can be standardized for production and can be administered in standardized doses. However, he also emphasized that receiving monoclonal antibodies does not mean that it can be used directly in the treatment of other patients, because verifications such as preclinical and clinical Phase I healthy human studies are also required. It takes a lot of time and effort to produce such antibodies in large quantities. In general, it takes at least about one year from the discovery of this antibody to enabling such antibodies to be expressed in large numbers in high-yielding cells. This is very detrimental to responding to sudden outbreaks.
