THE National University Hospital (NUH) is investigating how an elderly woman died while recovering from successful heart surgery.
One doctor involved in her care has since resigned, and another has decided against pursuing specialist training to become a surgeon.
Confirming that it has convened a formal inquiry, the NUH promised to provide the family with a full and transparent account when investigations are completed.
A spokesman told The Straits Times: 'If we find any inadequacies or lapses in our processes, we will put them right. We owe this to our patients.'
In a letter to the family, NUH said that registrar Tay Jia Sheng had given 24 hours' notice and left the hospital. He was training to be a heart surgeon and was overseeing Madam Heng on the day before she died.
NUH said that the medical officer on duty - Dr Ng Hui Chong - has since decided not to train in surgery.
