A 16-year-old girl in British Columbia died of toxic shock syndrome related to her tampon use, a coroner found.

Sara Manitoski was found dead on an overnight school trip in March 2017 near Vancouver Island, with an unknown cause of death.

Her friends left the cabin that morning for breakfast thinking that she was still sleeping, but when they returned she was still in bed with her alarm going off. Staff and students, along with emergency responders, attempted CPR on Manitoski but she did not recover.

Now, over a year later, the coroner has determined that she died of toxic shock syndrome, based on the strain of staphylococcus aureus found on a tampon that was in her body. The report states that she also had other signs of TSS, but notes that tampons are not the only cause of the bacterial infection.

“We know there is an association. And again, it’s very rare,” Island Health medical health officer Dee Hoyano told CTV Vancouver Island. “Certainly we know a person needs to have this particular bacteria to get sick, and then there may be something with tampon use, maybe prolonged tampon use, that puts that risk higher for developing a more widespread infection.”

Toxic shock syndrome is extremely rare — in 2016 there were just 40 reported cases in the United States, and about half were not related to menstruating women, Dr. Mary Jane Minkin, an ob-gyn from Yale University and member of PEOPLE’s Health Squad, told PEOPLE.

But when TSS does occur, it develops from toxins already present in the body.

“The first is vaginal colonization with a strain of S. aureus, which can make the toxin; the second is production by the S. aureus of the toxin; the third is penetration across the vaginal epithelium of enough toxin to cause disease; and the fourth is a lack of adequate titers of the neutralizing antibody to the toxin,” Dr. JoAnn Pinkerton, executive director of The North American Menopause Society told PEOPLE.

And women of younger ages are more susceptible to the disease.

“Younger women are more likely to get TSS, possibly because of more exposure through tampons or barrier contraceptive use,” Pinkerton said. “It may also be because they haven’t developed the antibodies yet.”

To reduce the chance of developing TSS, Pinkerton and Minkin recommend swapping out tampons every two to three hours, and avoid sleeping in them overnight. Additionally, it’s best to use lower-absorbency tampons to reduce dryness, and to switch off between tampons and pads.

However, Minkin said that women should not be overly concerned.

“The tampons themselves are not contaminated with bacteria,” she said. “The nasty staph organisms are unfortunately carried by us, not the tampons. So I would encourage all women to pay attention to their health, but not to worry.”

It was supposed to be a fun overnight school trip, but 16-year-old Sara Manitoski never woke up the next morning — a sudden death that authorities blame on toxic shock syndrome.
The Canadian teenager died of TSS as a consequence of staphylococcus aureus, CTV News reported, citing a report by the Coroners Service of British Columbia. Tests on a tampon found inside the girl’s body detected the presence of the bacteria, the report said, though the coroner noted that while tampon use increased the risk for TSS, it was not the only cause of it.


Sara died in March of 2017, but the official report was only released to the public last week.

“Sara was the sweetest, happiest, most charismatic little girl who had had such a bright wonderful future ahead of her,” her sister Carli Manitoski told TODAY. Sara never expressed any concerns about TSS and tampon use, she added. The teenager loved to travel and explore the world.
Manitoski has posted a warning to others on her Facebook page: “Women need to be more educated on this subject, some of you might not even know what TSS is… My beautiful, incredibly healthy sister died because of this so please share, educate yourselves and be cautious whenever using tampons.”

Most women probably don't worry a lot about their tampons, but should they?
The incident happened at camp on Hornby Island, British Columbia, where Sara — who attended school in Courtenay, British Columbia — traveled for an overnight school trip, according to a fundraising page set up for her family. She had complained of stomach cramps before going to bed, her sister wrote on Facebook. Sara was found unresponsive in her cabin the next morning and could not be revived.

An autopsy found redness on Sara’s neck, upper arms, upper chest, lower abdomen and thighs, the Vancouver Sun reported. A widespread red rash that looks like a sun burn is one of the symptoms of TSS.
What is toxic shock syndrome?
TSS is a life-threatening complication of a bacterial infection. The earliest cases involved women who used tampons during their periods, but today less than half of cases are linked to tampon use, according to the National Institutes of Health.
TSS first garnered national attention in 1980 after an “epidemic” of cases that coincided with the introduction of super absorbent tampons, the Centers for Disease Control and Prevention noted. The number of cases dropped significantly by 1986 after the U.S. government began to require standardized labeling for the products. A decrease in tampon absorbency, tampon package inserts and greater awareness of TSS among women also drove down the number of cases, CDC said.

TSS now occurs in up to three per 100,000 menstruating women, the National Organization for Rare Disorders estimated. Menstrual TSS most commonly occurs in girls and women 15 to 25 years old who are using tampons.

Dr. Lauren Streicher, professor of obstetrics and gynecology at Northwestern University’s Feinberg School of Medicine, has only seen one case in her almost 30 years of practice.
“It’s extraordinarily, extraordinarily rare,” Streicher told TODAY. “I certainly would not tell my daughters or my patients that they should be concerned about using tampons.”
TSS can affect anyone — men, non-menstruating women and children — because it can also occur with skin infections, burns and after surgery, NIH said.
TSS symptoms include:
  • confusion
  • diarrhea
  • general ill feeling
  • headaches
  • high fever, sometimes accompanied by chills
  • low blood pressure
  • muscle aches
  • nausea and vomiting
  • rash
To lower the risk for menstrual toxic shock syndrome:
The government advises these steps:

  • Avoid highly absorbent tampons.
  • Change tampons frequently, at least every four to eight hours.
  • Only use tampons once in a while during menstruation, alternating with pads.
Still, Streicher doesn’t advise her patients against the overnight use of tampons.
“It’s terrifying when you hear about [cases like Sara’s] because these are young healthy women,” she said.
“But when you look at the number of women who use tampons on a regular basis and keep them in overnight — this is one case. Think about the millions of nights that people spend with tampons in place… what this tells you is it’s extraordinarily rare.”

Use common sense when using tampons, she advised. If you do have symptoms of TSS, treat it as a medical emergency and seek help immediately.

A teenager died on a school trip after suffering toxic shock syndrome suspected to have been caused by a tampon.

Friends of the blonde teen thought she was asleep, according to a report by the BC Coroners Service, CTV News reported.

They at first thought she was sleeping in but became concerned when she wasn’t roused by her alarm clock.
The coroner’s report found a strain of staphylococcus aureus on a tampon along with other symptoms of toxic shock – a form of acute septicaemia which is typically caused by bacterial infection from a retained tampon or IUD.
‘We know there is an association. And again, it’s very rare,” Island Health Medical Health Officer Dee Hoyano told CTV Vancouver Island.

While tampons are not the only causes of toxic shock symptom, experts have warned it is a possibility women should be mindful of.

In 2012, UK model Lauren Wasser was found face down on the floor of her apartment with a fever, kidney failure, and suffering a heart attack.
The then-24-year-old was on her period and had just changed her tampon when she began to feel unwell while texting in bed.
‘Just flu-like symptoms, I was feeling nauseous and my head was pounding,’ Lauren told Style Like U.

The fever progressed to a point where the model’s kidneys were failing.
‘Thank god there was an infectious disease doctor there [at the hospital] because as soon as they found me I was plummeting so bad they couldn't understand why a healthy, young 24 year old like me was dying,’ Lauren said.
She was later told she needed to have her leg amputated due to the seriousness of her condition.
Now, Lauren campaigns for more research into the use of tampons and their possible dangers.

綜合外媒報導,事發當時,莎拉先跟同學說自己身體不舒服,要回床上躺一下,結果同學吃完早餐後發現莎拉昏迷不醒,趕緊跟老師講。學校老師及救援人員雖然立刻幫莎拉進行心肺復甦術,但最終莎拉仍不幸身亡。

日前驗屍報告出爐,內容指出,從莎拉當時使用的衛生棉條中測出金葡萄球菌,證實她的死因為「中毒性休克症候群」。醫師霍亞諾(Dee Hoyano)指出,這是非常罕見的案例,如果有患者罹患這種疾病,很可能衛生棉條使用時間過長,導致更大範圍的感染。
TSS是一種少見但卻可能致命的全身性疾病,有點類似流感,容易出現的症狀包括:高燒噁心、嘔吐、腹瀉、頭暈、昏厥和迷失方向。
