Definitely possible if the charger not designed properly. For example, insufficient isolation between low and high voltage sections of the circuit, use of low quality components, etc. make sure you buy UL listed devices, not shit stuff designed/made/copied by dumb/unscrupulous tiongkok...
Internal body resistance (wet) is 100 to 500 ohms. Unless you stick two probes into the body across the heart so that there is no chance of the current favoring the bath water instead, nothing fatal will happen. Froggy is right, something else (e.g. husband) happened.
But not likely since it takes the path of least resistance. If the connector or cord was faulty, and if the adaptor didn't automatically shutdown on an overcurrent event, then the path of least resistance is across the connector pins or break in the cord. It would not want to take a detour...
Not only that, the chargers re not simple voltage sourcing devices, they negotiate the voltage to be delivered with connected devices, shut down on short circuits, etc.
