Not even the 4 heavenly king

He look even better now then before.
The full package of maturity, muscility and good looks, height, speech and accent.

Will @earthquake be like this decades later?

I will try. But it's actually not difficult. It's all about discipline in food intake and regular exercise...

To get the full package is something that is hard hard hard.
We might be very cautious about the food we take, the exercise we do, but we are still prone to old age common disease like sclerosis, osteoporosis etc that will prevent us from standing tall.

Yep. Being fit and got nice body doesn't mean I wouldn't get cancer heart attack or stroke. I have seen many fit people also mati suddenly just like that. Anyway I just did a comprehensive blood test yesterday. All clear from cholesterol, blood sugar, kidney function, etc etc...except I fail in MCHC. My red blood cells are packed with more hemoglobin than usual. Will need to ask doctor next week.... :frown:

You can ask AI.
Perhaps the first thing is to redo first.

When red blood cells (RBCs) are packed with more hemoglobin than usual, it means their mean corpuscular hemoglobin concentration (MCHC) or mean corpuscular hemoglobin (MCH) is higher than normal. This can happen due to several reasons:

✅ 1. Hereditary Spherocytosis

  • Most common cause of increased MCHC.
  • RBCs become smaller, rounder, and more concentrated with hemoglobin.
  • Associated with hemolytic anemia, jaundice, and splenomegaly.

✅ 2. Autoimmune Hemolytic Anemia (AIHA)

  • Especially warm autoimmune hemolytic anemia.
  • Damaged red cells become spherocytes, similar to hereditary spherocytosis.
  • MCHC may be mildly elevated.

✅ 3. Dehydration (Artifactually)

  • Causes plasma volume to drop, making blood appear more concentrated.
  • MCHC or hemoglobin concentration may falsely appear elevated.

✅ 4. Cold Agglutinin Disease (Lab Artifact)

  • RBCs clump at cold temperatures → machine misreads the cell size.
  • This causes falsely high MCHC due to technical error.
  • Can be corrected by warming the sample before testing.

✅ 5. Lab Error or Hyperlipidemia (Pseudohyperchromia)

  • Very high lipids, proteins, or paraproteins** (e.g., multiple myeloma) can interfere with hemoglobin measurement.
  • Not a true increase in hemoglobin within RBCs.

Useful Tests to Investigate:

  • Peripheral blood smear (look for spherocytes or agglutination)
  • Osmotic fragility test (for spherocytosis)
  • Coombs test (for autoimmune hemolysis)
  • Repeat CBC using warmed sample (if cold agglutinins suspected)

Summary:

CauseMCHC StatusNotes
Hereditary Spherocytosis↑↑True increase; cells are smaller and packed
AIHA (warm type)Mildly raised; spherocytes present
Dehydration↑ (pseudo)Hemoconcentration, not a true increase
Cold Agglutinins↑ (pseudo)Clumping affects machine reading
Lab Artifact↑ (pseudo)Hyperlipidemia or paraproteins interfere

Cannot be dehydration. Since weekend I drank a lot a lot of water until my blood test yesterday

Then I suspect was it the Nespresso kopi I drank when I woke up in the morning?..blur blur forgot it's a fasting blood test and I drank a cup of no sugar nespresso kopi o....

But internet says MCHC usually not affected by consumption of kopi

Next step is to know the risk.
For me I will redo a MCHC test before pursuing.

Having too packed red blood cells (RBCs) — meaning high hemoglobin concentration or increased mean corpuscular hemoglobin concentration (MCHC) — can carry different risks depending on the underlying cause. Here's what you need to know:

1.

  • RBCs that are overpacked or misshapen (like spherocytes) are less flexible.
  • They struggle to pass through small blood vessels, leading to:
    • Poor oxygen delivery
    • Risk of microvascular blockage
    • Tissue damage

2.

  • Overpacked cells are often structurally abnormal.
  • They break more easily as they pass through the spleen or circulation.
  • Leads to:
    • Hemolytic anemia
    • Elevated bilirubin → jaundice
    • Gallstones (from chronic hemolysis)

3.

  • Damaged or abnormal RBCs are removed by the spleen.
  • Spleen may enlarge and start destroying other blood cells too → pancytopenia risk.

4.

  • If hemoglobin or hematocrit is very high (like in polycythemia vera):
    • Blood becomes thicker (hyperviscous)
    • ↑ Risk of blood clots, strokes, heart attacks
    • Symptoms include headaches, dizziness, visual changes

5.

  • Ironically, even though there’s more hemoglobin, the abnormal shape or destruction of cells can mean less effective oxygen delivery to tissues.

Key Labs to Watch:

  • Hemoglobin and Hematocrit
  • MCHC and MCH
  • Reticulocyte count (high in hemolysis)
  • LDH, bilirubin, haptoglobin (markers of hemolysis)
  • Peripheral smear (for shape changes)

When to Worry

If MCHC is persistently high and you're seeing:

  • Fatigue
  • Jaundice
  • Enlarged spleen
  • Dark urine
  • Unexplained anemia or clots

You should investigate further — especially for conditions like hereditary spherocytosis, autoimmune hemolysis, or polycythemia vera.

You got this leesult from a full blood count test?
Also have to see the range lah. If just fail by a little I don't think it will be of much concern.
