SINGAPORE — A five-year-old girl who drowned in a condominium swimming pool could not swim or float even with the use of a swimming board, according to coroner findings released on Monday (18 October).

The girl had been left unsupervised in the pool after her mother returned home to use a toilet and prepare dinner.

When the mother returned, she could not find her daughter. She enlisted the help of a condominium security guard, who found the girl at the bottom of the 1.2-metre adult swimming pool on 8 October last year. The girl was conveyed to hospital and was later found to be brain dead. Her parents decided to take her off life support to end her suffering.

The girl was pronounced dead at 11.06pm on 11 October 2020 at KK Women and Children’s Hospital. An autopsy found the girl’s cause of death to be from drowning, with her only other injuries found to be consistent with resuscitation efforts. She was then 1.06m tall and weighed 18kg.

Delivering her findings on Monday, Coroner Kamala Ponnampalam said that the girl likely had difficulties trying to float with her swimming board or had slipped off the narrow ledge of the swimming pool when she was walking along it. She then panicked and began gulping water before sinking to the bottom of the pool, according to the coroner.

She found the girl's death to be a tragic misadventure and reiterated the need for children to be supervised by adults while at swimming pools.

"This incident is yet another stark reminder that children are impulsive and unpredictable. Even the most compliant child is often given to curiosity. The young child also lacks the maturity to fully understand the dangers present," said Ponnampalam.

"In the case of a swimming pool, a child may not appreciate how swiftly she may be overcome if she should inadvertently fall into the deep waters. There must always be an attentive adult present to warn and prevent an unfortunate incident."

A notice board at the side of the swimming pool stated that there was no automated external defibrillator or lifeguard on duty at the condominium.

Girl only had three swimming lessons before death​

Before her demise, the girl had had three swimming lessons with a coach at her condo pool on 24, 28 September and 5 October last year. She was a beginner who could not swim or float in water.
The coach had taught the girl how to float with a pool noodle. The girl was interested to try floating using a swimming board, but did not manage to maintain her balance in the water. The coach advised her not to use the board until she could float confidently. He described the girl as shy and reserved who would panic while floating unassisted.

The coach testified that when children struggle in a pool, they end up sinking fast due to the amount of water they gulp. According to the coach, the girl might have tried using the board but lost her balance.
Since moving to the condo, the mother testified that she had twice taken her daughter to play in the pool area. On one occasion, the woman had gone to the gym while her daughter remained in the pool area with another child who was supervised. She knew that her daughter could not swim or float as she had attended only three swimming lessons.

On the day of the incident, 8 October last year at around 6pm, the girl returned home from kindergarten and was keen to go to the swimming pool. She changed into her swimsuit. Her mother wanted to start preparing dinner but did not want to disappoint her daughter.

The woman brought her daughter to the ground floor of the condominium and the girl went into the 0.8m deep jacuzzi equipped with a blue swimming board at around 6.03pm. The mother did not enter the pool.

After observing that there was an adult couple present in the area, the mother, who wanted to use a toilet, told her daughter that she would return home. She felt comfortable leaving her daughter at the pool due to the presence of the couple. But she did not ask the couple to keep an eye on her daughter.
The mother then returned home. After using the toilet, the woman decided to reheat some soup for dinner as she did not think it would take her long.

She returned to the pool at about 6.31pm but could not see her daughter. She sought the assistance of a security guard but they did not see the girl.

Minutes later, the guard discovered her at the bottom of the swimming pool facing down, and jumped into the pool to bring her out. He lay the girl on the sun bed and began blowing into her mouth. The mother began crying, attracting the attention of a nearby resident, who came to perform cardiopulmonary resuscitation on the girl.

CCTV footage of the pool area showed that before her death, the girl had been walking alone around the pool with her swimming board. She attempted to float at the shallow edge of the pool at one point while holding onto the board.

At around 6.19pm, she was recorded heading to the back of the pool with a board. A minute later, a small movement could be seen in the water, with the board spotted floating in the water seconds later. The board floated away and the movement subsided.

The father raised the question of whether the man had been interrogated in relation to the case, and suspected that someone might have pushed his daughter in.

The investigation officer said that she had already contacted the man, and he had explained that he had come in from the condominium's back gate and headed to his apartment. The man claimed to have not seen the girl nor the board.

However, the father wanted clarity on how long it would take the man to walk from the back gate to the pool, given the timings indicated by different CCTV footage.

To allow further investigation, the coroner's verdict on the child's death has been postponed to a later date.

A five-year-old girl drowned in a condominium pool after her mother left her unattended for about 20 minutes to head home for a toilet break.

The incident happened on Oct 8, 2020, at The Asana condominium located at Queen's Road.

In a coroner's inquest on Friday (May 5) to determine the cause of death, the investigation officer and two other witnesses were brought to the stand, reported Lianhe Wanbao.

The investigation officer told the court that the mother had brought her daughter to the pool for a swim at 6.10pm.

But not long after, the mother went back up to their apartment to go to the toilet, leaving the girl at the pool by herself.

The mother also spent some time in the kitchen preparing dinner for the family. She returned about 20 minutes later.

According to surveillance camera footage, the girl was seen walking along the side of the swimming pool, from the main pool to the jacuzzi pool and back again.

At about 6.20pm, CCTV footage of a partially-obscured area of the pool captured some splashes on the surface of the water. Following that, a blue-coloured kickboard was seen floating away. The investigation officer surmised that that was the moment the girl fell into the pool.

The mother reappeared at the poolside at 6.31pm, based on CCTV footage. She circled the pool several times searching for her daughter before frantically approaching the security guards at their post for help.

A security guard spotted a body face down in the pool and dived in. He placed the unconscious girl on a deck chair by the pool. A resident, who came out to help upon hearing the mother's distraught cries, performed CPR on the girl.

The ambulance arrived and took her away to the hospital, but she never regained consciousness. After three days, the girl's parents made the heartbreaking decision to take their daughter off life support.

According to investigations, the girl was unable to keep herself afloat as she wasn't proficient in swimming. She had only attended three swimming classes prior to the incident.

The depth of the pool where the girl was suspected to have drowned was 1.2m while she was 1.06m tall.

The mother had shared that on previous occasions where she brought her daughter swimming, the latter usually enjoyed playing in the kids' pool, but would head to the jacuzzi pool area when she saw other children playing there.

The mother indicated that on one of those occasions, she watched her daughter from the gym. Even though the view of the jacuzzi was obstructed from where she was, she trusted that her daughter would be safe because there were other adults in the pool.

The coroner's verdict on the child's death has been postponed to a later date after the child's father indicated in court that he had additional queries regarding the report.

He raised the question of whether a Caucasian man, who was seen in the CCTV footage, had been interrogated during investigations. He put forth a question if the girl could have been pushed into the pool.

The Causcasian man was seen in the same frame walking up to his apartment shortly after the kickboard floated away.

In response, the investigative officer said she had contacted the man for an interview. The resident in the condominium had said that he did not see the girl or the board, and had just entered the condominium from the back gate to go up to his apartment.

The officer did not subject him to further questioning as she felt he was unrelated to the case.

However, the father indicated that he would like to ascertain the time it took the man to walk from the back gate of the condominium to the pool, as the two video surveillance cameras showed different timings.

The coroner agreed to allow further investigations to be conducted, postponing the verdict to a later date.

SINGAPORE - The death of a five-year-old girl who drowned when she was left alone at a swimming pool has been ruled a tragic misadventure.

Delivering the findings of the coroner's inquiry into the death of Aurelia Liu Jiabao, Coroner Kamala Ponnampalam said on Monday (Oct 18) that no foul play is suspected in the girl's death.
Her family was not in court to hear the findings.

The coroner called the incident "yet another stark reminder that children are impulsive and unpredictable".
"In the case of a swimming pool, a child may not appreciate how swiftly she may be overcome if she should inadvertently fall into the deep waters," said Coroner Kamala, who added that an attentive adult must be present to warn children and prevent an unfortunate incident.

The court heard that Aurelia had taken three swimming lessons, but her swimming instructor had earlier testified that she could not swim or balance in water.

At about 6pm on Oct 8 last year, the girl, who was 1.06m tall, and her mother, Madam Lu Ying, went to the swimming pools at their condominium, The Asana, near Farrer Road.

There was an adult couple in the 1.2m-deep swimming pool and Aurelia splashed about in the 0.8m-deep Jacuzzi while her mother watched.

At about 6.10pm, Madam Lu needed to use the toilet and told her daughter that she was going up to their flat.
Madam Lu said that she felt comfortable leaving the child alone in the pool as there were adults nearby. She did not ask them to watch the girl.

As seen on closed-circuit television footage, the couple got out of the pool about a minute after the woman left.

The footage shows Aurelia with a blue swimming board in the Jacuzzi and then walking along the edge of the pool before sitting on the shallow edge. She then lays on her stomach and tries to float.

At about 6.19pm, the girl can be seen walking towards the other side of the swimming pool holding the swimming board.

About a minute later, small movements are seen in the pool water at the front of the swimming pool. Then, the blue swimming board floats across the water and seconds later, the water is still.

Madam Lu returned to the pool at about 6.31pm and could not find her daughter. She asked the security guard for help.

At about 6.35 pm, they discovered Aurelia submerged face down at the bottom of the pool.

A resident performed cardiopulmonary resuscitation on Aurelia and an ambulance arrived soon after. Aurelia was unconscious when she was taken to KK Women's and Children's Hospital.

Her family was informed that she had a pulse but was brain dead. They removed her from ventilator support on Oct 11 last year.

An autopsy found that the cause of Aurelia's death was drowning.
