Some interesting observations about the photo.
1) The "table" with the cake does not appear to be a table. It is made up of 2 parts and highly polished. The size of the table makes it the correct size to serve meals to a patient in bed.
2) The objects on the table is interesting.
a) There are 2 cups on the table which would be appropriate to serve tea.
b) There are 3 paper napkins, each placed in an exquisite in a napkin holder. The attention to place a paper napkin in a high class napkin holder would be consistent with serving the refreshment to VIPS.
c) There are 2 folders on the two tables. One is face down while the other if face up. The one which is face up has some kind of design. The resolution of the photo does not however allow us to see the design in greater detail. The fact that there is a design and not stock paper folder suggest an event/briefing of some sort.
d) There are two face down packets of what appear to be pens in plastic casing for the taking notes.
Hypothesis:
The photo was taken in a hospital (most likely SGH). The VIPs had gathered for some form of briefing/discussion with LKY. The state of the items on the table suggest that the briefing has been concluded. After the briefing was concluded, there was an impromptu birthday cake and photo op. This photo was not meant to be published. For reasons unknown HSK chose to share it on this facebook. Given the sensitivity of LKY's health, the decision was made to photoshop the picture to remove references that it was taken in a hospital. The quality of the photoshop is extremely low and is suggestive that it was done by a political volunteer rather than a professional internal team.

Good observations, but I think everything was photoshopped in this picture. that includes him sitting down and also the cake itself. If this is an impromptu photo taking session, than the wording on the cake should be facing him. Instead, its faced outward to the camera. I believe that someone had the cake made and than edited into the photo. when you place a birthday cake infront of the birthday boy, you always have the wording facing him. the size of the plate on which the cake sits is way out of proportion to the table.
Notice too that not many people are actually staring at the camera when the picture is taken. Obviously, the camera person would say cheese, or something to attract everyone's attention to him. But in this case, maybe 2 people are actually looking at the camera. the four persons standing up on the right of the picture is obviously photo shopped. If you take a picture of a birthday party, the money shot is not this shot. the money shot should be the one of Old fart actually cutting the cake. that should be the picture that heng Swee kiat put out. But it wasn't. that tells me that the whole thing is a fraud. Old fart is probably dead already. gay Loong busy hiding all his assets to keep a low profile.

When two tables join, the legs of the tables would be obstructing LKY's legs if he sat at that position.
The photo is definitely faked... you don't even need to fiddle with the contrast, just observe for yourself the official bigger photo:
