【陳麗婷、許佳惠、地方中心╱連線報導】台大醫院重大疏失造成五名病患被移植愛滋器官事件,至昨已有一名換腎患者得知真相,當場情緒潰堤。而在成大換心的女病患,本要在術後要與男友完婚,昨卻被證實體內驗出捐贈者的愛滋病毒抗體,極可能被感染。

台大醫院上周三摘取腦死邱姓男子器官,檢驗愛滋呈陽性,但卻告知台大與成大移植團隊是陰性,致使台大、成大各有四名、一名病患,被移植入愛滋器官。事後兩院僅告知病患家屬,沒告訴患者,僅有一名台大換腎病人由家屬告知真相。

換腎男得知後崩潰

台大醫院感染科主治醫師洪健清昨說,這名換腎病人得知後情緒崩潰、大喊無法接受,已派社工安撫。而在該院進行移植的四名病患,院方已進行抽血檢驗,但因要檢測病毒量才能確定是否感染愛滋,因此暫不公布初步檢驗結果。

另一名在成大換心的三十多歲女子,原計劃術後與男友完婚,卻因台大疏失被植入一顆愛滋心臟,一切被打亂,消息傳來,女子男友痛哭,老母親更是淚水缺堤,沒人敢告訴還躺在加護病房的女病患。

昨成大再傳出這名換心女子,體內已驗出愛滋病毒抗體。成大移植團隊成員李伯璋表示,女子移植前體內並無愛滋病毒抗體,代表身上愛滋病毒抗體來自捐贈者,是否感染,還需檢測病毒量。

主刀醫師內褲染血

關懷愛滋患者的關愛之家協會秘書長楊捷說,這五名患者有知道真相權利,醫療團隊應對他們坦白,「該面對就要面對」。除五名病患家人痛苦,參與移植手術的台大、成大醫護也是一片低氣壓。成大醫院人員指出該院團隊前往摘心時,主刀醫師全程靠在手術台,台上不斷溢出混合灌流液與愛滋患者的血水,手術結束該醫師不但下半身被血水浸濕、內褲被染紅,另名醫師襪子也被血水染紅。洪健清也指一名台大移植團隊醫師手術時,曾被邱男體液噴到臉部與眼鏡。

成大醫院指二十六名參與移植的醫護人員血液檢驗報告昨出爐呈愛滋病毒抗體陰性,但因愛滋有三至六個月空窗期,還需追蹤。台大則指已陸續通知醫護人員進行抽血檢驗。衛生署醫事處處長石崇良說,已要求台大與成大關注患者、家屬與醫護身心,若醫護過於沮喪應暫調職務,以免影響醫療品質。

愛滋病毒小檔案

病毒:人類免疫缺乏病毒(Human Immunodeficiency Virus,HIV,俗稱愛滋病毒)

傳染方式:性行為、血液傳染、母子垂直感染等

潛伏期:指感染後到出現臨床症狀的期間,一般是5-10年

國內現況:去年新增1796例感染者,近3年感染途徑以男性間性行為為主

預防投藥方法:疑似遭感染者首次抽血若驗到抗體,不代表已遭感染,而是帶有傳染者本身抗體,需預防性投藥1個月後追蹤病毒量,至少觀察3個月甚至更久,若抗體消失且無驗到病毒,表示沒遭感染,若持續都有抗體且驗到病毒,表示已感染,需終生服藥

預防方法:避免危險性行為、勿共用針頭、加強婚前健檢及產前愛滋篩檢

Health minister distressed over flawed organ transplants

Taipei, Aug. 30 (CNA) Department of Health (DOH) Minister Chiu Wen-ta said Tuesday he felt deep regret and distress over operations at local hospitals in which organs from a single HIV-infected donor were transplanted into five patients.

Chiu, who returned to Taipei Tuesday morning after cutting short a visit to the United States, said the DOH will organize "care teams" to help patients and medical personnel involved in the flawed transplants.

"We will give priority to the transplant patients while also offering counseling and concern to members of the transplant teams," Chiu told reporters upon his arrival at Taiwan Taoyuan International Airport.

The teams will consist of social workers, volunteers, and mental health and medical service experts, Chiu said.

The DOH will investigate possible human error at National Taiwan University Hospital (NTUH) and National Cheng Kung Univeristy Hospital (NCKUH) in performing the transplants that put the five recipients and medical personnel at risk of contracting HIV.

"We will get to the bottom of what caused the flawed transplants," Chiu said.

The DOH will also keep close tabs on follow-up treatment, and provide both preventive anti-AIDS medication and other therapies for the transplant patients, Chiu said.

Shih Chung-liang, director of the DOH's Bureau of Medical Affairs, confirmed Monday that the two hospitals would submit their account of the incident to the agency by Aug. 30 and that the DOH would come up with a formal investigative report on the case within 14 days.

The flawed transplants -- four by NTUH and one at NCKUH -- were performed on Aug. 24. The hospitals only realized two days later that the donor was an HIV-carrier.

Prior to the operations, the NTUH transplant team relied only on a phone communication to get the results of HIV tests on the organs, and it thought it was given a green light when in fact the organs had tested positive for the virus.

NCKUH took the NTUH transplant team's word that the organs were clean and went ahead with its heart transplant.

NTUH admitted on Aug. 27 that its medical team did not follow standard operating procedures in the case and should have checked for the test results on the computer before proceeding. (By Pien Chin-feng and Sofia Wu) enditem/ls

One of Taiwan’s best-regarded hospitals said H.I.V.-infected organs were mistakenly transplanted into five patients after a staff member misheard the donor’s test results over the telephone. The five are now being treated with anti-AIDS drugs, an official at the National Taiwan University Hospital in Taipei said Monday. The hospital said on its Web site that the mistake occurred because a staff member mistakenly believed he heard the English word “nonreactive” concerning the donor’s standard H.I.V. test. The donor’s heart, liver, lungs and two kidneys were transplanted into separate patients on the same day.

TAIPEI - Taiwan's local health authorities Monday announced that three special teams will be formed within two days to thoroughly investigate a medical accident in which hospitals used the organs of an HIV-positive donor in transplant surgeries.

The Taiwan University Hospital transplanted the organs of an HIV-infected donor on Aug. 24, putting five organ transplant recipients at high risk for contracting AIDS.

Taiwan's health authority Monday issued a statement on its website saying that scholars, law experts, as well as representatives from social organizations would be invited to form the three special mission teams.

The team in charge of investigating the cause of the accident should investigate the entire series of events leading to the accident and determine any loopholes. Punishment will be given according to the results of the investigation, said the statement.

The victim assistance team will visit the organ recipients, their family members, as well as the donor's family relatives in order to calm them and provide help.

The medical consultation team will provide the victims with medical consultation and constantly monitor their conditions.

Taiwan's health authorities have urged the Taiwan University Hospital and Taiwan's Cheng Kung University hospital to not only closely observe the health of the five organ recipients, but also to appropriately handle their family members' anxieties.

Health authorities also required the two hospitals to monitor the health of medical staff involved in the transplant and provide them with necessary assistance.

If the two hospitals should have to suspend their organ transplant operations, authorities will arrange for sending relevant patients to other hospitals to receive organ transplants, said the statement.

Prior to the dangerous transplants, it was learned that a 37-year-old man had suffered a serious head injury and was sent to the Nanmen Hospital in the city of Hsinchu for medical treatment.

As his condition was not promising, his family took the initiative to contact the Taiwan University Hospital to arrange for the donation of his organs without knowing that he was HIV-infected.

The man's organs were later brought to Taipei to be transplanted into five different patients, but medical staff failed to check the donor's medical records.

Four organ transplants were done in Taiwan University Hospital, while one was conducted in Cheng Kung University Hospital.

Upon completing the transplant surgeries, medical staff pulled the donor's medical records and only then learned that he was HIV-positive.

This accident marks the first time that organ transplants might lead to recipients contracting AIDS in Taiwan.
