If the investigating authority finds out that they conducting their own investigation by calling up staff that are involved it opens up the pandora box of pain. No issues if it is normal accident with no death. Thats the reason why Straits Times corrected their press release because they knew that SMRT had no idea what was going on. Companies are normally charged in courts when investigation reveals criminal negligence or breach of any safety law etc. Can you imagine they conducing their own investigation. What do you their outcome will be?

The correct procedure is they do an internal investigation but MOM or SPF will do their investigation separately. Each have their own term of reference eg SPF will determine any foul play.. Etc.
