It's not fake, becoz it was never intended to be actual 'waterboarding' (that's just a convenient adjective). That the fat mamasan and those gangsters want to torment, abuse and punish her is real. That they repeatedly and violently assault her until she bleeds (near the end of the clip), that's real. That she's traumatized, screaming and crying, that's real.

The only question is, if anyone here understand what they're saying Japanese, can you figure out the backstory? What did she do that made them want to torture her? Refuse to serve a particular customer? Refuse to participate in a particular sex act (eg. eat shit) in a porn film?
