BANGALORE - India's indigenously produced Covid-19 vaccine is at the centre of a controversy after it was licensed with clinical trials for effectiveness still incomplete.

Covaxin was granted emergency approval on Sunday (Jan 3) by the Indian drug regulator. Also getting the nod was AstraZeneca-Oxford University's Covishield, to be made in India by Serum Institute, the world's largest vaccine producer by volume.

The decision to approve Covaxin, developed and manufactured by Hyderabad-based Bharat Biotech, triggered a row. The Indian health minister said it was granted permission for "restricted use in emergency situations" in the "larger public interest".

The Indian drug regulator said Covaxin's inactivated virus formulation would be effective protection against mutant strains.
Health experts, however, said there was no data in the public domain to support any such claims.

Covaxin has completed only two of three required phases of clinical trials. The first two ensured that the drug is safe. The third one that tests for efficacy began only in late November.

Dr Gagandeep Kang, vice-chair of the board of Coalition for Epidemic Preparedness Innovations, a global partnership seeking to develop vaccines, said she had seen nothing like this approval before.

"This is hypothetical at best," she told CNBC-TV18.

On Sunday, Serum Institute chief executive Adar Poonawalla told Indian channel NDTV: "There are only three vaccines with proven efficacy - Pfizer, Moderna and Oxford/AstraZeneca. Everything else has been proven to be safe, just like water."

Serum Institute's Covishield has completed all three trial phases.

The Indian government approved it based on AstraZeneca's interim data from the British trial, which showed 70.42 per cent efficacy, although there were doubts about dosages given.

In a virtual press meeting on Monday, Bharat Biotech chairman Krishna Ella slammed critics. "We are a global company operating in 12 countries, including the US and UK, and have a proven track record of 16 vaccines," including for the Zika virus, typhoid, chikungunya and rotavirus, he added.
"Why are Indian companies and scientists bashed? If other companies can get approvals based on just 100 people's data, why not me?" He was referring to Covishield's data submitted to the Indian regulator.

Asked if Covaxin's approval was premature, he quoted the health ministry's clinical trial rules that allow accelerated authorisation for vaccines based on "remarkable efficacy" shown in phase two trials.
He said Covaxin was licensed based on its efficacy in trials involving animals, proven "platform technology" (the manufacturing mechanism) and robust immune responses to multiple viral proteins in the 375 and 380 people who got two doses in the first two phases.

In the past month, many sites conducting the Covaxin trials admitted to delays due to shortages of volunteers. Bangalore's Vydehi Institute of Medical Sciences and Research Centre aimed to administer the first dose to 1,000 people but only about 500 turned up.

In Bhopal, activist Rachna Dhingra alleged about 700 people were administered the first dose of Covaxin without informed consent.

They are from poor neighbourhoods adjacent to People's Hospital, which is running the trial, and many said they were told they were getting the vaccine, not the trial vaccine.

The investigator at the Bhopal hospital refused to answer questions, and Bharat Biotech's media adviser said the company was not aware of the developments.

Experts fear hasty approvals, incomplete trials and a lack of transparency could discourage people from getting vaccinated. A scientist in Bangalore admitted: "I for one will skip vaccination."

The drugs prescribed by the doctors to cure a person’s disease are unfortunately adulterated most of the times. According to the World Health Organisation (WHO), 35 percent of the fake drugs sold all over the world comes from India and it occupies the counterfeit drug market of nearly Rs 4,000 crore. 20 percent of the drugs sold in India are fake. Drugs prescribed for cold and cough or a headache are mostly either fake or of poor quality.

A patient receiving an authentic drug for his disease has become a challenge these days. The unfortunate thing is that the administration is mostly aware of everything that is happening. According to the data produced by the Department of Food Safety and Drug Administration, more than 10 percent of the counterfeit drugs have been introduced into the market and 38 percent of the drugs are not effective as they are of low quality.
According to the work report submitted by the Department of Food Safety and Drug Administration, Uttar Pradesh, in the year 2015, around 5,150 drugs and 301 cosmetics product samples seized in the raid conducted in the last eight months by them were sent to the laboratory for investigation. The examination report of 4,723 of these samples was astounding. 506 of these drugs were found to be fake which establishes that more than 10 percent of the drugs found in the market are fake. Apart from this, only 2,902 of these samples were found to be effective as per the standards.
National Institute of Biologicals conducted a study across the country during 2014-2016 following the orders of the Ministry of Health and Family Wel. During the survey, there were about 47,954 samples collected from the government hospitals, dispensaries, and pharmacies. More drugs of poor standards were found in the government hospitals as compared to the pharmacies in the market. While pharmacies in the market had 3 percent of the poor quality drugs, government hospitals had 10 percent. There were around 0.023 and 0.059 fake drugs found in the retail outlets and government hospitals respectively. According to the survey, there has been some improvement in the situation over the time.
Most of the fake drugs are made in extremely polluted atmosphere and the demand for these drugs is present all over the world, from South Africa and Russia to our neighbouring countries such as Myanmar and Nepal.
The standards created by the Pharmacy Council of India for setting up of pharmacy shops are also being regularly violated. One of the rules is that there should be a gap of at least 300 metres between two pharmacies. However, it is a common sight in India to spot two or three pharmacies right next to each other. The regular violations, which are the result of government’s apathy or corruption, are proving to be harmful to the common people of India.

SINGAPORE - Travellers from Indonesia and India will be able to enter Singapore under a quarantine-free travel scheme from Nov 29, in a move that will rebuild air links with two of Changi Airport's top three markets.
In addition, travellers from Qatar, Saudi Arabia and the United Arab Emirates will be able to enter Singapore from Dec 6, in a further expansion of the Vaccinated Travel Lane (VTL) scheme.
Indonesia has yet to open its borders for quarantine-free travel from Singapore.
But travellers can travel without quarantine to and from India, Qatar, Saudi Arabia and the UAE, which earlier opened their borders for vaccinated travellers from Singapore.
More details about the VTLs will be announced soon.
The Civil Aviation Authority of Singapore (CAAS) announced the VTL expansion on Monday (Nov 15).

It advised travellers to check entry requirements of the respective VTL countries, which may vary. The requirements could also change as the Covid-19 situation evolves, the CAAS added.
The CAAS and Ministry of Health are closely monitoring the Covid-19 situation worldwide and in each VTL country, and will adjust measures if needed, it said.
Transport Minister S. Iswaran said during a press conference by the Covid-19 multi-ministry task force that the countries Singapore will open up to are all significant destinations.
For example, India accounted for about 7 per cent of passenger arrivals at Changi Airport in 2019.

India, Indonesia, Qatar, Saudi Arabia and the UAE are in Category Two of the Health Ministry's Covid-19 risk classification for border measures.
The countries' classification means they either have similar or lower Covid-19 incidence rates than Singapore and other VTL countries.
By Dec 6, Singapore will have a total of 21 VTLs.
The Republic had earlier announced VTLs with 16 other countries. Thirteen of these are already in effect, and three more with Malaysia, Sweden and Finland are due to start on Nov 29.

On Monday, Trade and Industry Minister Gan Kim Yong said the land border between Singapore and Malaysia could reopen in a few weeks' time under a "Vaccinated Travel Lane-like arrangement".
Travellers entering Singapore under the VTL scheme do not have to serve a stay-home notice. Instead, they have to test negative for Covid-19 two days prior to departure for Singapore, as well as upon arrival.
Since Nov 11, travellers can submit a negative test result from either a polymerase chain reaction (PCR) swab test or a professionally administered antigen rapid test.
Upon arrival, they will be required to take a PCR test, and can go about their activities if they test negative.
With the addition of the new VTL countries, the overall daily quota of arrivals via the scheme will be increased from 6,000 to 10,000.

The authority said that as at 11.59pm on Sunday, it has issued a total of 45,380 Vaccinated Travel Passes for short-term visitors and long-term pass holders from 13 countries.
These countries are Australia, Brunei, Canada, Denmark, France, Germany, Italy, the Netherlands, South Korea, Spain, Switzerland, Britain and the United States.
Short-term visitors and long-term pass holders have to apply for the Vaccinated Travel Pass to tap the VTL scheme. Singaporeans, permanent residents and children aged 12 and below do not have to do so.
As at Sunday, 24,070 VTL travellers have entered Singapore.
These VTL travellers comprise 13,274 short-term visitors or long-term pass holders, 9,595 Singapore citizens and permanent residents and 1,201 children aged 12 years and below.
