I dont think it is trivial.

These people have no respect for the rules.

I face this often myself. Patients wanting to have special treatment against the rules and dont listen to their doctor dont respect what the doctor needs to do in order to prescribe.

What good is it for one to accommodate the customer and then end up losing their licence to operate or practice? No customer is worth that in reality.
