【星島日報報道】(星島日報報道)香港大學醫學院昨公布,發現全球首宗由大鼠傳染人類的戊型肝炎個案,估計傳播途徑為進食有大鼠糞便污染的食物引致。該名去年接受肝臟移植手術後染病的五十六歲男病人,居於黃大仙彩雲邨,經服藥後已康復。港大醫學院微生物學系教授袁國勇表示,在該病人身上驗出由大鼠感染人類的「C物種戊型肝炎」,暫未能確定是否該名患者的免疫系統較差而染病,還是老鼠帶有的病毒基因變異而感染人類,但此個案已響起警號,不排除將來會有社區爆發,呼籲市民不能掉以輕心,保持家居及街道清潔。
一名五十六歲住在彩雲邨的男子,患有乙型肝炎,去年五月接受屍肝移植手術,七月肝功能出現異常,肝酵素指數持續上升,測試發現他並非患有較常見的「A物種戊型肝炎」(HEV-A)。瑪麗醫院再為他檢驗,九月確診他對「C物種戊型肝炎」(HEV-C)呈陽性反應,該物種病毒一直只在大鼠身上,從未發現可感染人類,今次是全球首宗發現。男病人隨即服用「利巴韋林」藥物,肝功能漸回復正常,今年三月身體亦再無該物種病毒。
港大醫學院微生物學系教授袁國勇昨稱,檢驗過捐肝者組織及手術用具,均沒有戊型肝炎病毒,病人手術後亦沒有出現排斥,故排除由移植手術感染,相信他是進食有老鼠糞便的食物所致。袁補充,患者居於低層單位沒有發現老鼠,但其單位鄰近垃圾房,不時有老鼠出沒,但暫未發現該處的老鼠帶有「C物種戊型肝炎」,反而瑪麗醫院在覆檢二○一二至一七年間,在黃大仙收集的二十七個老鼠樣本中,發現其中一隻帶有該病毒,但基因排序跟患者所染的病毒稍有不同,未能確定是否同一源頭。
袁國勇說,以往從未發現「C物種戊型肝炎」可感染人類,但難以確定是因為該病人身體虛弱、病毒基因改變,導致更容易感染人類,還是以往已有感染個案但從未發現。他解釋,以往只有「A物種戊型肝炎」感染人類,故全港均無對「C物種戊型肝炎」病毒進行測試,今次也是瑪麗醫院專門為是次個案制定相關檢驗,建議醫管局在不同醫院也發展該類測試。
袁國勇又稱,戊型肝炎患者病徵通常為食欲不振、嘔吐等,潛伏期一般達五至六個星期,大部分人服藥便可康復,但有肝藏問題、免疫系統較差及孕婦等人士感染,可致死亡。他補充,戊型肝炎主要透過飲用遭到糞便污染的食水而傳播,其他則為食用未經煮熟的受病毒污染肉類、透過輸入受感染的血液製品,以及由孕婦傳給胎兒等。衞生防護中心亦建議,市民要保持良好衞生習慣,如進食前及如廁後洗手,避免飲用不明的及進食未煮熟食物等。
他強調,應採取謹慎態度,因為或許個案是病毒「大爆發前的訊號」,更直言「香港老鼠數目未必低過香港人口」,僅在隱藏位置不常見。袁又指,○三年沙士,特別留意街道清潔,但近年後巷十分骯髒,市民不能全依賴食環署人員清潔街道,每人亦有責任保持地方乾淨。
