A friend asked me to help out as he is stationed overseas and needed to get this form done for him as a proxy.

I was amused by the announcement made (dated Oct 2010: http://www.spf.gov.sg/epc/cncc_notice.htm):

"Why is there a change in the policy for the CNCC?
Police are not obligated to issue CNCC but have done so as a form of international cooperation since other countries provide similar documents as well. However, this service does not form part of police’s core function. Further, following the recent differentiation approach between citizens and non-citizens in terms of benefits in the healthcare, medical and education sectors, a similar distinction was also drawn with regard to CNCC issuance policy. As such, CNCCs will only be issued to citizens henceforth. The policy change will affect only a small number and there are alternative mean such as applying for a Statutory Declaration through the Commissioners of Oath or qualified legal practitioners."

I wonder how many other government forms contain these strange permutations - one common observations by friends who've had to apply for holiday visit visas for Singapore is that they are shocked that they have to declare their Race and Religion!

I think they just change Qns 24, added a few more options to choose from.
I remember mine don't have so many options.
But they still refuse to put in an option for: Because of PAP
Anyway, your friend have to be there himself to complete the application.
