The Health and Manpower ministries will review if improvements are needed to better ensure that foreign workers in Singapore are vaccinated against diseases such as diphtheria, said the Ministry of Health (MOH) yesterday.
The review follows the death of a 21-year-old Bangladeshi construction worker who developed fever and swelling of the neck - symptoms of diphtheria - on July 30, and died last Friday.


The ministry said it was alerted to the case last Thursday. It is the first local infection of the contagious bacterial disease here in 25 years.
At present, foreigners do not need to provide evidence of previous vaccination against the disease upon arrival in Singapore as diphtheria is included in the childhood vaccination schedule of most countries, said MOH.


The ministry stressed that most people here are vaccinated against the disease, or have already built an immunity to it, so the chances of coming down with diphtheria are low. "The disease is rare in Singapore and the threat of spread is low due to good vaccination coverage of the local population," said the MOH spokesman.
"As the (worker who died) had not travelled out of Singapore recently, he is likely to have been infected in Singapore. Investigations are ongoing to determine the source of the infection."



The last local case of diphtheria was reported in 1992, and the last imported case here was in 1996.
MOH also said that the two individuals who were in close contact with the worker and had developed sore throats - a symptom of diphtheria - continue to be isolated in wards at Khoo Teck Puat Hospital (KTPH). MOH did not reveal the nationalities of the patients.
Both are in stable condition and have been given preventive medication and a booster diphtheria vaccine, along with the other 46 people identified for further checks. Two Singaporeans were among those treated. The respiratory samples of all 48 people have been taken for testing and the results are pending.
The worker, who lived in a dormitory in Yishun Avenue 7 and worked at Teban Gardens, developed a fever and swelling of the neck on July 30. He went to KTPH the next day, where he was immediately isolated and hospitalised, but died last Friday. A respiratory sample taken from him tested positive for the bacterium causing diphtheria.
Diphtheria is a contagious disease that causes inflammation of the mucous membranes, and could result in fatal heart and nerve damage. Treatment involves administering diphtheria antitoxin, as well as antibiotics.

The solution is for Singaporeans to be more self reliant. The men should build their own skyscrapers and swept their own backyards while their women look after the offspring. Banglas and maids will no longer be needed.

You could have misdiagnosed the cases that you did come across because you had no experience with the infection.

When I first arrived in NZ a Singapore doctor friend who was over for a winter holiday diagnosed my chilblains as a "fungal infection". No Singapore doctor will see a case of chilblains while running his HDB practice.

Of course, we do not hear of chilblains here...but that doesn't mean that, they are people here who don't get them. Those in the F&B industry or food processing or those in contacts with cold temperature or ice, all the time, do come down with it. But, as a general, no...for we do not have that cold or chilly weather here.

Of course, it will be costly to check for all types of diseases for all the workers coming in, but those from the 3rd world...the list must be updated all the time, to accommodate those dieases we have controlled or eliminated through health policies or vaccinations here, but prevalent in those countries, like TB, Dengue.."chicku..whatver"...etc..

Like I have mentioned, we have opened a "pandora box"....of course, we have to be vigilant....we can do that much...but a responsible government must do ore..other than, have eyes to make money only...:rolleyes:

In some conditions accurate diagnosis is not that important. For instance with dermatology type conditions you either decide if it is an infection? Then what organism? Fungal culture? Pus culture? Or just treat empirically with antibiotics? Inflammatory without infection? Topical Steroids. Then there are specific conditions that might suggest systemic illness often autoimmune. Eg SLE Malar rash. Raynauds. Psoriasis. Cancer? Biopsy? Acne? topical vs oral treatment. Rosacea? Special rashes eg HFMD? Kawasaki? Age group is important Skin treatment wise there isn't much variation really in types of therapy. More often than not steroids. Then watch. If not better biopsy.

So with cold injuries really it is just rewarming and prevent infection be it fungal or bacterial. Being able to make an accurate diagnosis sometimes is really just academic if he treatment is the same thing.

If it is diphtheria I will surely miss for sure the diagnosis but I will not miss the fact a patient is really sick and needs specialist opinion.

GP is not expected to solve everything. But important to filter out what is normal or simple and no need see specialist or if complicated to point to right specialist.

If you leave to patient have seen patient with cough go to respiratory specialist then say nothing then go ENT then say nothing then finally go gastroenterologist say is GERD. Waste their money and time.

They dunno mah. Or got back pain Chinese relatives say maybe is kidney problem (TCM) so to nephrologist do all test say is normal then finally is just back muscle strain or facet joint arthropathy.

No doctor can diagnose everything and treat everything or know everything. You go specialist they just whack everything they know in their specialty if cannot then tell you sorry dunno. Go another specialist.

Problem is sinkie system expect GP to know everything and treat everything or else say they are useless. That is why invariably health care costs going to go sky high in singapore when there are no more GP. And no doctor want to be GP anymore.
