The health declaration form is found on the website of the Department of Social and Health Development, which delivers a copy of it to all residents and visitors. The Health Declaration Form comes in paper format, and it is encouraged that you get this form from the bureau. You will be able to check whether you already have health insurance, and you can keep on using the healthcare facilities supplied by the bureau and cover for your medical insuranceplan.
